Bupa Accused Of Bullying Tactics Over Physiotherapist's Fees Which Will Restrict Patient Choice

Bupa has set in motion a campaign to lower the fees that physios charge their members, and to reduce the number of approved providers, which will decimate the industry and restrict patient choice.

Physios should have the commercial autonomy to set their own fees and to request that their patients settle their accounts following treatment.

Bupa are trying to reduce the number of Physios holding approved provider status from the current number of 13,000 practitioners to 1000, thereby severely restricting patient choice.

Facing an outlook of rising costs and falling revenue, Bupa is exercising its considerable corporate muscle to coerce Physiotherapists, in private practices all over the country, into absorbing core cost-bearing functions. Bupa has admitted using its dominant market position to force local Physiotherapists to take over the major share of its claims processing in an effort to transfer costs and increase its own margins.

Bupa originally stated in discussions with the Chartered Society of Physiotherapy (CSP) and the Private Practitioners Association (Physio First) in October 2008 that it wanted to cull the number of physios it recognized to treat its members from 13,000 to 1000. Bupa have since retreated from this position in discussions with the CSP but not before they had caused widespread panic in the industry. Private physiotherapy is largely made up of sole practitioners or small businesses. In the current economic climate, the threat of losing a significant proportion of income is of grave concern to physiotherapists.

Bupa's attempt to try and impose onerous, one sided, contracts on vulnerable practitioners will inevitably disenfranchise patients. The tough contractual terms being proposed will require a substantial increase in time spent on unproductive, unpaid administration on Bupa's behalf. The result will be less time spent with the patient and a significant deterioration in the personal bond between patient and Physiotherapist that is essential to speedy recovery.

Diabetes-Specific Exercise Tips

• Discuss with your doctor what types of exercise might be appropriate for you. Complications of diabetes such as severe eye disease and nerve damage may make some forms of exercise dangerous for you. Your doctor may also schedule a test to see how your heart responds to exercise.
• Do not exercise if your blood sugar is greater than 250 mg/dL (milligrams per deciliter) and your ketones positive. This is an indication that you already may have a lack of insulin and exercise will only cause a greater rise in your blood sugar. Hydrate yourself and adjust your insulin as necessary, contact your health care provider.
• Use caution when exercising if your blood sugar is greater than 300 mg/dL without evidence of ketones, exercise may help decrease your sugars, but it's possible they will increase instead. Hydrate well prior to and after exercise and keep track of your sugars and ketones.
• Learn the effects of various types of exercise on your blood sugar.
• Have carbohydrate-based foods available for exercise and for the period following exercise. Add carbohydrates to your meals if you plan on doing exercise, adjust your insulin dose appropriately in anticipation of exercise.

Diabetes and Exercise Tips

• To reduce the risk of hypoglycemia if you have diabetes, follow a regular routine of exercising, eating your meals, and taking your medicines at the same time each day.
• Prolonged or strenuous exercise can cause your body to produce adrenaline and other hormones that can counteract the effects of insulin and cause your blood sugar to rise. If you are participating in strenuous exercise (exercising at your maximum capacity) or prolonged exercise (lasting for several hours or more), your insulin and/or oral diabetic medicine may need to be changed. Talk to your health care provider about how to adjust your medicine.
• Be careful exercising when your medicine is reaching its peak effect.
• Depending on the time of exercise, reducing your dose of either long-acting insulin or short-acting insulin will be necessary. Your doctor can recommend how to make this adjustment.
• Exercise with someone who knows you have diabetes and knows what to do if you have a low blood-sugar reaction.
• Wear a medical identification tag (for example, MedicAlert) or carry an identification card that states you have diabetes.

What Types of Exercise Is Best for Diabetes?

Strength Training and Type 2 Diabetes

The latest findings show that exercise such as strength training has a profound impact on helping people manage their diabetes. In a recent study of Hispanic men and women, 16 weeks of strength training produced dramatic improvements in sugar control that are comparable to taking diabetes medication. Additionally, the study volunteers were stronger, gained muscle, lost body fat, had less depression, and felt much more self-confident.

Aerobic Fitness and Type 2 Diabetes

Any activity that raises your heart rate and keeps it up for an extended period of time will improve your aerobic fitness. Aerobic exercise helps decrease the risk of type 2 diabetes and helps those with diabetes to better manage their blood sugar levels. Besides the health benefits, exercise is fun and boosts your mood. It's hard to feel stressed when you're walking fast on a treadmill or swimming laps in a pool.

Exercise Affect Blood Sugar Levels

Normally, insulin is released from the pancreas when the amount of sugar (glucose) in the blood increases, such as after eating. Insulin stimulates the liver and muscles to take in excess glucose. This results in a lowering of the blood sugar level.

When exercising, the body needs extra energy or fuel (in the form of glucose) for the exercising muscles. For short bursts of exercise, such as a quick sprint to catch the bus, the muscles and the liver can release stores of glucose for fuel. With continued moderate exercising, however, your muscles take up glucose at almost 20 times the normal rate. This lowers blood sugar levels.

But intense exercise can have the opposite effect and actually increase your blood glucose levels. This is especially true for many people with diabetes. The body recognizes intense exercise as a stress and releases stress hormones that tell your body to increase available blood sugar to fuel your muscles. If this happens to you, you may need a little bit of insulin after intense workouts.

For a variety of reasons, after exercise, people with diabetes may have an increase or a decrease in their blood sugar levels.

Exercise helps control type 2 diabetes

• Improving your body's use of insulin.
• Burning excess body fat, helping to decrease and control weight (decreased body fat results in improved insulin sensitivity).
• Improving muscle strength.
• Increasing bone density and strength.
• Lowering blood pressure.
• Helping to protect against heart and blood vessel disease by lowering 'bad' LDL cholesterol and increasing 'good' HDL cholesterol.
• Improving blood circulation and reducing your risk of heart disease.
• Increasing energy level and enhancing work capacity.
• Reducing stress, promoting relaxation, and releasing tension and anxiety.

Meeting of FCRP-BD at Mirpur Centre

A second meeting of Friends of CRP-Bangladesh (FCRP-BD) was held at the Mirpur Center of the Centre for the Rehabilitation of the Paralysed (CRP) on 2nd May 2009 to form a structured FCRP-BD body. .
The meeting started with the welcome speech of the founder and coordinator of CRP Valerie A. Taylor. With the aspect of the first meeting in March the Chairman of the Trust for the Rehabilitation of the Paralysed (TRP) Mr. Saidur Rahman started the discussion regarding a structured form of the FCRP-BD. He mentioned that in the last meeting we started discussing the possible ways that the FCRP-BD members could contribute at CRP. He said 60% contribution come from the individual supporters of CRP. If we could include the corporate donors and supporters, who especially related with the media can do a greater help for CRP. He stressed on professionalism. He advised the Donor Liaison officer of the organization Mr. Abu Zafor to share the list of FCRP-BD members with the other members who were present at that meeting, So that they could work on a small group to form a FCRP-BD body. After one month they will sit again. Mr. Rahman said, so far we could not utilise the government officials; we should think to seek help from them properly.
Mr. Mustaque Ahmed a long standing supporter of CRP said, FCRP-BD should sit together within very short time to form a body. They may even sit once in every two or three months. He mentioned that in Savar centre, we charge a minimum fee from the patients for treatment according to their financial condition. A patient is catagorised according to his/her economic status and received the subsidise treatment. He suggested to provid absolutely free treatment for all the patients.
Another FCRP-BD member Mr. Aziz U Ahamed, a formerly of UNHCR(Geneva) said, 425 registered members are enough to make CRP a billion dollars project. He stressed that FCRP-BD should be the fundraising force of CRP. He suggested enlisting the potential persons who can contribute at CRP but not yet listed.
The meeting decided to sit again in a large gathering after working out with the FCRP-BD member-list within very short time to form a structured body for FCRP-BD.
FCRP-BD members Mr. Asgar Hosain, Dr. Mohammad A. Jabbar along with Mrs. A. Jabbar and some senior staff from Savar and Mirpur were present at the meeting and participated the discussion.

The Bangladesh Health Professions Institute (BHPI)

About CRP Bangladesh

CRP has a strong commitment to training and believes that training plays a vital role if health care services in Bangladesh are to be strengthened and developed. At present, only a few health professionals in Bangladesh are trained adequately to provide appropriate treatment and rehabilitation.

In response to the lack of training facilities for health care professionals, BHPI was established in 1992 with the goal of producing highly skilled staff to work in health care provision and rehabilitation countrywide.

BHPI provides a number of nationally recognised training courses. These courses include Physiotherapy, Occupational Therapy and Speech and Language Therapy Bsc Honours Degrees, Physiotherapy and Occupational Therapy Diplomas, Laboratory Sciences and Radiography Diploma, Certificate in Education for Special Education and Diploma in Rehabilitation Nursing. All the courses run at the BHPI are recognised by their respective government departments.

Some other institutes in Bangladesh offer physiotherapy courses at various levels but the BHPI is the only institute in the country to offer a Bsc Honours Degree course in Occupational Therapy and a Bsc Honours Degree course in Speech and Language Therapy. CRP is constantly promoting these professions by lobbying and networking with the government to create posts and awareness of the value of allied health in the medical community. However, one of the best ways for people to become aware of the value of these professions is to see them in practice. Successful examples of this are the student placements in different government hospitals and non-government hospitals such as Dhaka Medical College & Hospital (DMCH), National Institute of Cardio Vascular Diseases (NICVD), National Institute of Traumatology, Orthopaedic and Rehabilitation (NITOR), National Institute of Mental Health (NIMH), Combined Military Hospital (CMH) Armed Forces Institute of Pathology (AFIP) and Bangladesh Institute for the Rehabilitation of Diabetes Metabolic and Endrocine Disorders (BIRDEM). Even though the students are not fully qualified, they make a tremendous impact and can demonstrate how effective the multidisciplinary team can be.

From the occupational therapy placement at NIMH, CRP has received interest form different institutions and organisations who are interested in creating positions in the future, including Drug Rehabilitation Centres and the Schizophrenia Association.

World Physical Therapy Day - Bangladesh

he Bangladesh Physiotherapy Association (BPA) celebrated World Physical Therapy Day on 8 September 2008 with a press conference, attended by the Chairman of the regulatory reform commission. The BPA used this press conference to recommend the restructuring of the health care delivery system in Bangladesh, and inclusion of qualified physical therapists in the health care system. There was also a rally in central Dhaka which was attended by around 700 people.

Colourful posters were on display in physiotherapy institutes, medical colleges and hospitals to raise awareness about physiotherapy among the general population.

Physiotherapy course in Bangladesh was first started at the National Orthopedic Hospital (NITOR) under the affiliation of the University of Dhaka in 1973.From then it follows a path of rise and fall. Within last 15 years, realizing the prospect, some private universities have also launched the subject though have lack of facilities.
Introducing a subject does not mean the establishment of the respective profession.
BSc physios don’t get job at public hospitals cause there is no post for them. We repeatedly told the govt to create physio posts at the govt funded hospitals. But the authority did not show any interest regarding this, mysteriously {may be the doctors who think we would reduce their income, influenced the government .Doctors here have strong association which is politically biased}.
After graduation we begin our career. Actually, private health sector is doing a great job by proving jobs for physios.
I believe, continuing professional development by gaining knowledge, conducting research, is essential if we want to see our profession in a respectable position. But there is no opportunity for further education in Bangladesh. We are insisting our govt to start MSc course at public universities. Apart from these efforts, support from international level is also crucial.
In short, we need 3 things: job, council, further education. We all know what the problems are. Now we need motivated action to solve the problems.
